Supplemental Nutrition Assistance Program (SNAP) is a federally funded program that offers nutrition assistance to millions of eligible, low-income individuals and families. The program is run by the US Department of Agriculture (USDA). If applying for Illinois SNAP benefits, a decision on your eligibility will be made within 30 days. If determined eligible, SNAP benefits will be issued from the date the application is filed.
Follow the directions on the form. Type in as much information as you can. If you can't answer all the questions, that's ok. You must include your name and address. You may print out the application and write on it if you prefer. You must sign the form. Once you've completed the application, carry, mail or fax it to your local Family Community Resource Center.
